Please click on any of the links below to download, view, and print the forms. After you have printed them, you may fill them out and send or fax them to us. Monthly Expenses Worksheet: This is a very useful worksheet that helps you estimate your total monthly living expenses and compare those costs to what you will pay in an assisted living environment.

Admissions Assistance

Prior to the time you establish residence, we will arrange an appointment with you to complete the admissions forms. To assist you, we request that we be allowed to obtain the following information, which may also be required by other health care providers.

Please bring copies or the original of:

  • Social Security Card
  • Medicare Card
  • Power of Attorney or Guardianship Papers
  • Insurance Cards
  • Living Will

Clinical Services Evaluation.

Residents are required to be evaluated to determine the appropriate level of services to be provided, if any.

If you do not have a local physician or dentist we will assist you in finding one.

Please inform us of your choice of personal physician, dentist or other health care practitioner who will serve you. If you do not have a local physician or dentist we will assist you in finding one to serve you while you are a resident at Park Vista Retirement Living.
